I don't have the original on this one. This is as I received it about a year back. Validity is subject to someone recognising it. The questions and the syntax are about right. Came from a disk. I wish I had the form - it'd be 40 pages long!

I don't remember posting this before, although I did post it to Fidonet in the old days.
